6.B. Investigator-Initiated Academic Research

The Department of Respiratory Medicine, Indira Gandhi Government Medical College, Nagpur, actively undertakes investigator-initiated research addressing critical challenges in tuberculosis care, spanning prescription practices, treatment safety, and programmatic implementation.

These studies are conducted independently without external funding, reflecting a strong academic commitment to generating clinically relevant and policy-oriented evidence. Collectively, they provide real-world insights aimed at improving the quality of tuberculosis management in India.

1. PRAMAN India Study

(Prescription Review and Meta-analysis of Anti-tuberculosis Treatment in Non-programmatic Settings in India)

The PRAMAN India Study is a first-of-its-kind systematic review and meta-analysis evaluating anti-tuberculosis prescription practices in non-programmatic settings across India over three decades (1990–2024). The study synthesises evidence from multiple regions to assess the prevalence, patterns, and determinants of inappropriate tuberculosis prescriptions.

It provides nationally representative evidence on deviations from standard treatment guidelines, highlights variations across provider types and regions, and identifies critical gaps in treatment practices. The findings offer important insights for strengthening tuberculosis care, improving prescription quality, and informing policy and programmatic interventions.

The study was presented at the Asia Pacific Region Conference of the International Union Against Tuberculosis and Lung Disease (APRC 2026), Bangkok, Thailand (5 February 2026) by Dr. Gyanshankar Mishra, reflecting its relevance to regional and international tuberculosis discourse.

2. Linezolid Safety Study in Drug-Resistant Tuberculosis

Adverse Drug Reactions due to Linezolid in the Programmatic Management of Drug-Resistant Tuberculosis in India: A Retrospective Multicenter Study

This investigator-initiated, multicenter study evaluated adverse drug reactions associated with linezolid use in patients with multidrug-resistant and rifampicin-resistant tuberculosis under programmatic conditions in India. Conducted across multiple tuberculosis units, the study provides real-world evidence on the safety profile of linezolid in bedaquiline-based regimens.

A substantial proportion of patients experienced adverse drug reactions, particularly peripheral neuropathy, often requiring dose modification or discontinuation. The study highlights the importance of early detection, active drug safety monitoring (aDSM), and rational use of linezolid in programmatic settings.

3. INH Mono-Resistant Tuberculosis Study

Treatment Outcomes of Isoniazid Mono-Resistant Tuberculosis under Programmatic Management in India

This investigator-initiated study evaluated treatment outcomes of isoniazid (INH) mono-resistant tuberculosis using a standardized regimen under programmatic management conditions in India. The study assessed real-world effectiveness of treatment strategies in routine healthcare settings.

The findings demonstrated favorable treatment outcomes, supporting the effectiveness of standardized regimens for INH mono-resistant tuberculosis within national programmes. The study also underscores the importance of appropriate drug resistance testing and systematic monitoring in optimizing treatment outcomes.

The study was presented at the World Conference on Lung Health 2023 (The Union), Paris, France by Dr. Radha Munje.

6.C. Community Projects

Active Case Finding of Tuberculosis in Nagpur City: A Multidisciplinary Collaborative Project (NIIDAN TB Project)

Institution: Indira Gandhi Government Medical College and Hospital (IGGMCH), Nagpur

Period: 23 July – 31 December 2025

The Department of Respiratory Medicine actively participated in the NIIDAN TB Project, a large-scale, multidisciplinary initiative aimed at early detection and control of tuberculosis in Nagpur.

A total of 25,283 individuals were screened through verbal assessment, of whom 10,689 were identified as presumptive TB cases. Among these, 7,624 individuals underwent AI-assisted chest X-ray screening, yielding 1,238 AI-suspected cases.

Subsequently, 1,376 sputum samples were collected, with 1,195 tested by RT-PCR, resulting in the detection of 53 confirmed TB cases. Contact tracing identified 169 household contacts, of whom 139 underwent Cy-TB screening, leading to the identification of 7 additional Cy-TB positive cases.

The Department played a key leadership and operational role in this project. Dr. Radha Munje served as Chairman, Dr. Gyanshankar Mishra was a Core Working Group member, and Dr. Kapil Mate and Dr. Sadaf Khatib were part of the College-Level Supervisory Committee.

The project was executed in collaboration with multiple departments, including Microbiology, Radiology, and Preventive and Social Medicine (PSM), highlighting a strong multidisciplinary approach to tuberculosis elimination.
